Supervisor Resources
Below you will find forms and resources pertaining to your role as a supervisor. Please note that there are some details that pertain to all supervisors across programs. If you have any questions regarding anything pertaining to these forms please contact the administrative team at education@tufts.edu.
- W9/IRS documentation is no longer collected on a paper form. If this is your first time serving as a supervisor, or if it's been more than three years since serving, please indicate such on your agreement form. An invitation will be sent from a system called "Paymentworks" to collect the necessary information needed to pay supervisors at the end of the academic year.
- The Department of Education is migrating away from paper forms and will be attempting to collect Agreement Forms, Invoices, Logs, etc. via a Qualtrics form.
